FORMS for HIGH SCHOOL STUDENTS:

Once all forms have been completed, please bring them to your school’s main office. Detailed instructions below.

  2. Concussion FormThe Grand Forks Public Schools is providing concussion management information as required by North Dakota State Law (NDCC 15.1-18.2). The statute requires that each student and their parent/guardian shall document that they have viewed information regarding concussions that may be incurred by students participating in athletic activities. This form must be signed and returned to the school office prior to the athlete beginning practice. A concussion form must be submitted each year that an athlete participates.
  3. Code of Conduct FormParents/Guardians and the student-athlete need to sign the Code of Conduct form. This form must be signed. 
  4. Fee Waiver Request Form

Participation Fees

The Grand Forks Public Schools charges students participating in athletics, speech, debate, and drama a participation fee. We are requiring athletes to turn in their physical cards, concussion forms, pay their participation fees, and check eligibility in the Main Office.

The procedure will work like this:

  1. Each high school student is to report to RR’s Main Office anytime and bring their signed physical form, signed concussion form, signed code of conduct form, and the appropriate fee. Office hours are 7:00 A.M. – 4:00 P.M.
  2. Office personnel will collect the above items and check the eligibility list.
  3. If everything is in order, the athlete will be issued an activity “white” card which is to be taken to the coach.

Coaches will NOT collect physical forms, concussion forms, code of conduct forms or money, it will all be done in the Main Office. If the athlete voluntarily quits after nine practice days have taken place, there will be no refund.

  1. The participation fee for all athletics is $75.00 per activity.
  2. Fees may be waived in cases of undue hardship. The District Athletic Director will determine hardship cases using criteria specified in the waiver form and will process all waiver requests. Waiver forms are available in the Main Office and at the link below.
  3. The fee is to be paid prior to the opening of practice. Fees are to be paid to the secretary in the Main Office in the school in which the student attends.
  4. Student managers, trainers, statisticians, dance team members, and cheerleaders on an athletic team will not be required to pay a fee.

FORMS for MIDDLE SCHOOL ATHLETES:

  • Any MIDDLE SCHOOL student interested in participating in cross country/track first must bring their completed forms and $45 payment to the main office at their middle school. Once you have the white card from your middle school, you must bring it to Coach Dafoe/Coach Bakke.  If your child is ONLY running Middle school Cross Country, the fee will stay at $45. However, if they are going to run any JV or Varsity races/meets, they must pay an additional $30 fee and complete the High School Code of Conduct Form. Talk to Coach Dafoe (XC) or Coach Bakke (Track) if you are unsure. 

Physical Examination

The North Dakota High School Activities Association now requires a physical examination every year. Only the final page of the form will be turned in to the school office by the student-athlete.  The physical must be taken on or after April 15th to be valid for the next school year. This form must be completed and returned to the school office prior to the athlete beginning practice. In addition to the options listed below, athletes may choose to see their primary care provider.

  • Tuesday, August 7 | 5 – 7 p.m.

*No appointment necessary for Altru Family Medicine Residency dates. Walk-in only.

  • Tuesday, July 17 | 6 – 8 p.m.
  • Thursday, July 26 | 6 – 8 p.m.
  • Thursday, August 2 | 6 – 8 p.m.

Immunizations will be available during these sports physicals.
